So, the toe next to the big toe needed straightening. So he drilled a hole in to my bones and inserted a wire through the toe bones and in to my foot to get it to align correctly. And then he reattached ligaments and repaired the plantar plate.

The part of the x-ray which is pointed forward is a set screw which kept a protective ball in place to keep the end of the wire protected.

The pea looking thing was a ball which covered the end of the wire sticking out of toe.

Well, he had to remove some gout damage too. This in spite an MRI last summer being read by a radiologist as negative for gout.

My foot was a mess.

Maybe the radiologist read the Cliff's Notes version - pretty poor.

Hope the recovery/rehab goes well.

That was one of the issues with them deciding on gout. The pain is similar to what you described and I never had the swelling and redness in the toes. Mine was on top of my feet.

Yes. You can get killed in a car crash with a seat belt at 30mph. And survive another without it at 40 mph. If you want to maximize your chances of surviving a car wreck, wear a seat belt. If you want to minimize your chances of getting gout, weigh 130, not 230. I am an actual doctor FYI. You really gonna argue with me? And sorry for the preaching. Anesthesiologists dislike people that go from 130 to 230. If you had gout at 130 you probably have a genetic predisposition. But what's with the 230? It's easier to keep you safe at 130 than 230. If you eat more, walk more.

Doc, I gotta stand with Temery on this only because it’s really hard to go from 230 to 130. I don’t know his height but what about a change of diet and exercise if lacking to be fit at 200 lbs with good blood and blood pressure numbers? As a doctor you know how out of shape the American population is and how bad their diets are. Just take a walk through Target or Walmart.

I just injected one of my patients with Kenalog/Lidocaine into his first metatarsal-phalangeal joint immediate and lasting relief

When you say lasting relief, how long would you say it lasts for? When my podiatrist gave me the Kenalog in to the MTJ he said it might be something he'd have to do every 6-9 months and that's why he recommended surgery.

Kenalog injections that I have performed have lasted 3 to 12 months, likely 5-6 on the median.

I didn’t know that’s what gout was. Not making fun, just a little shocked.

I have now learned more about gout. I knew it impacted the foot and was painful. I also had some idea of how one develops gout. But also am to much of a pessimist to believe it is only related to diet, has to be genetics as well. But I never knew it could be specific as impacting one toe.

Lately I have gotten into the habit of picking up stray leaves that appear on the bottom of my pool between services with the two big toes of my right foot. And of course once you become a certain age if you do anything repetitive consistently with any body part that body part will start to hurt.

Now when I get out of bed the first step on my right foot creates a pain wake up that stinks. Used to rely on my morning coffee to get me going.

The comment about getting old is not for sissies is so true.

The only positive is I made an appointment with my podiatrist. Nicest MD I have ever had.

That's where my bunions, plantar plates and LCL's in the foot came in to play. While my primary care doctor and podiatrist both say I have symptoms of gout, all I know is since my surgeries on my foot earlier this year, there has not been a single flare up in my foot or toes. It was almost two years of constant pain to isolate root cause and finally do the foot surgery.
Basically a bunion angles your big toe inward and that pushes your other toes outward. That misalignment puts strain on the ligaments connecting your toes to your feet and on your plantar plates on the pad of your foot. Do enough repetitive work on them and you can cause strains or tears which causes flare ups.

It took me waiting for a flare up and getting an MRI done at that time to see the extent of the damage on my feet/toes.

Do you know if you have an actual structural issue? I had very high arches since I was a kid. Orthopeadists and Podiatrist had me with custom orthotics from teen years until I finally ditched them in my late 30s.

My own research, reading papers etc. led me to conclude that unless you have a specific, real biomechanical issue, those insoles just prevent your feet and ankles from building the strength they need. They're a crutch. Switching to mostly zero drop shoes and ditching the insoles has fixed almost everything for me, although if you do it too fast you can have Achilles issues. Also the high heel-toe drop shoes many wear shift a lot of stress to your knees. Lower drop shoes finally fixed my knee issues and hip issue.

So by all means, find a good one. But too many of them don't like to consider ways to really improve your feet and just prescribe custom orthotics.
