If the "15" is for 15 years since our first NC, I'm not getting the "3." Of course I know we've had 3 NCs (and now five Final Fours) during that stretch, but shouldn't the point be that this would be our 4th NC in 15 years?

If you use the Logo you are going to run into licensing issues. Without it you could crowd source that shirt today using various sites available online to do it. No charge for the design

How about H15TORY REPEATS ITSELF

Where the vertical portion of the P is a 1 and the E is a 3. Some color scheming would have to be done to make sure it stands out, but I think that would look cool
