Probably not the best one, but one that sticks out for me:

When the firm in Mad Men merges with the brits and that one hot shot British ad guy gets his foot run over by the riding mower. Took me hugely by surprise at the time.
 
How about most of the stuff that happens on Legion. I gave up on that show right before the end of season 2.
 
For recent stuff, I was catching up on Designated Survivor.

Season 2, First Lady, getting T-boned and killed by a driver who was texting, while in her motorcade was pretty far out of left field.

Mike Myers and Kanye West were together doing a pitch for a Hurrican Katrina benefit. Suddenly West blurts, "George Bush hates black people!!". Myers had no idea how to react.
 
Mark Greene getting the snot kicked out of him on ER. That came out of nowhere.
I forgot about ER. Here's another good one.

I had read the books, but for those that hadn't, the Red Wedding was a "what the " just happened moment.

Watching this alone, I stood up, hands on head and said WTF out loud. Definitely what popped into my mind at the thread title.

The 2nd one, also GoT, is the Viper vs. the Mountain.
 
Since I just watched it, has to be The Sopranos final scene.
 
Learning that John Lennon got shot from Howard Cosell on Monday Night Football.

Well if real life events count, I was just about to leave for work one morning, my roommate had the tv on w/ national news, and the World Trade Center collapsed. I sat transfixed to the tv for about an hour, was late for work that day.
 
To mix it up a little bit, when Ryan got the promotion in the Office at the end of season 3. That came completely out of nowhere.

In Breaking Bad, when Hank finds Walt's copy of "Leaves of Grass." That scene was crazy because you KNEW being the end of the mini-season that something big was going to happen, and the scene played so peacefully, with the family happily barbecuing on a Sunday afternoon. I was waiting for someone to come busting in to attack Walt for getting out of the game. So tense. And then Hank sits down to take a crap. Awesome stuff.

2nd plane hitting the tower was definitely the most WTF moment of my lifetime. I wasn't awake for the 1st, but I do not believe it was televised live.
