This is mainly due to just enough OTA signal bleed-in that advertsisers and affiliates want to include them. Again, it's almost exclusively cable only - there have been attempts for satellite to join suit, but it's never really taken hold, unfortunately. Streaming providers similarly only give you one DMA. It's also not perfect - some of the local affiliates will fight to get SV channels removed, so they get exclusivity.
