I used the school copy machine to turn a paper form into a pdf. Now I want to use this to create a form with text boxes so I can type entries.

The forms were printed up years ago, and we just reorder more - it no longer exists on a computer anywhere for me to do this more easily.

Any idea how I can get this done?

You just need some software that has OCR (optical character recognition) capabilities. Like Zen stated above, Adobe has a solution ... or sometimes the scanning software you use also has the option when scanning.

I've tried OCR programs. Always get garbage as output.

Some of that is to be expected based on the quality of the original. It really doesn't like headers and footers. Usually a quick spell check handles 80% of the errors. Still faster than retyping.

I am downloading a trial version.
Okay in the pro, there is a "Forms" button. Just click on that and use the Form Wizard. It should be pretty straight forward.
