Junior Naphessa Collier fouled out versus Texas on 16 Jan 2018 (boxscore).

It probably happens more often than you think, though not as much as it used too.

And as I posted on another thread, I believe Meg fouled out in less than 6 minutes.
She got foul #1 with about 2 minutes (2:09 mark???) left in the 3rd and got #5 before the 6 minute mark of the 4th.

I was at the game and as far as I could tell (and folks who watched at home with big TVs can let me know how wrong I am), two of those were phantom fouls. Maybe one, if I’m being generous.
